The patchy nature of the deficits means that what’s lost is specific — and what’s preserved is real.
Cooking together rather than taking over the kitchen. Walking side by side rather than supervising. Music, conversation, gardening — activities where their preserved abilities can show up. The goal is to build care around what remains, not only around what has been lost. This matters for dignity. It also matters clinically — engagement and social connection are documented protective factors against further cognitive decline.
Managing the stepwise pattern
The key planning challenge in vascular dementia is that the next step down is unpredictable but almost certainly coming. Have a care plan ready — who steps in if your parent suddenly can’t live alone after the next event? Don’t wait for the crisis. - More of: leafy greens (minimum 6 servings/week), other vegetables, berries, nuts, olive oil, fish (minimum 2 servings/week), beans, whole grains, poultry
- Less of: red meat, butter and margarine, cheese, pastries and sweets, fried food
- Alcohol: if consumed, moderate — the evidence for alcohol in vascular dementia is not as supportive as sometimes claimed in popular media
Sodium reduction is particularly important given the central role of blood pressure. Reducing processed food significantly reduces sodium intake without requiring rigid dietary restriction.
Hydration matters for blood viscosity and blood pressure regulation. Dehydration can precipitate confusion in vascular dementia that looks like progression but resolves with rehydration. Regular fluid intake through the day — not just when thirsty — is worth monitoring.

Aerobic exercise — particularly brisk walking — reduces both the risk of vascular events and the rate of cognitive decline in established vascular dementia. A 2025 systematic review found that 150 minutes per week of moderate aerobic exercise produced measurable improvements in cognitive function in people with mild vascular cognitive impairment.
What works in practice:
Walking is the most accessible and evidence-supported form of exercise for this population. The target of 150 minutes per week — about 20-30 minutes most days — is the clinical recommendation. Falls risk is real and must be managed

- Home safety assessment and modification

Medication — what families need to know
The primary medications in vascular dementia are cardiovascular, not cognitive.
Blood pressure medications, antiplatelet drugs (aspirin, clopidogrel), anticoagulants (for atrial fibrillation), statins, and diabetes medications are the core pharmacological treatment. The goal is preventing the next vascular event.
On cholinesterase inhibitors (donepezil, rivastigmine): These are sometimes prescribed for vascular dementia, particularly in mixed dementia with an Alzheimer’s component. The evidence is weaker than for Alzheimer’s and not all guidelines recommend them for pure vascular dementia. Ask the neurologist explicitly about the evidence for the specific prescription being recommended.
Antidepressants: Depression is extremely common in vascular dementia — more common than in Alzheimer’s, and partly biological rather than purely reactive. SSRIs are generally safe and effective and can improve quality of life and functional outcomes. Blood pressure medication timing: Some blood pressure medications cause orthostatic hypotension — a drop in blood pressure when standing that can cause dizziness and falls. The research landscape for vascular dementia has historically lagged behind Alzheimer’s, but this is changing.
SPRINT-MIND findings: The landmark SPRINT-MIND trial demonstrated that intensive blood pressure treatment (targeting systolic BP below 120 mmHg rather than 140 mmHg) significantly reduced the risk of mild cognitive impairment and probable dementia. And it is genuinely tragic when families don’t — when they watch blood pressure go uncontrolled, medications get missed, and exercise stop — not out of negligence, but out of not knowing that these things matter differently in vascular dementia than in other conditions.
