20 Minutes of Walking Daily — The Single Most Evidence-Backed Intervention for Dementia

More research supports daily walking for cognitive health than almost any medication. Here's the evidence, and more importantly, how to actually make it happen with a parent who has dementia.

If you could give your parent one intervention with the strongest evidence base for slowing cognitive decline, it would not be a medication. It would be 20 minutes of walking daily.

This is not a wellness claim. It is the conclusion of multiple large randomized controlled trials and systematic reviews conducted over the past decade.

The evidence: The EXERT trial (2022, published in JAMA) found that 52 weeks of aerobic exercise — primarily walking — significantly slowed cognitive decline in adults with mild cognitive impairment compared to a stretching control group. The effect size was meaningful, not marginal. Multiple subsequent studies have confirmed and extended these findings.

Why walking specifically: Walking is weight-bearing, requires coordination and spatial navigation, is social when done with others, and can be performed at whatever pace is appropriate. It has a dramatically lower injury risk than other aerobic exercise forms and requires no equipment or facility access.

The real challenge — making it actually happen:

People with dementia often resist scheduled activities. The 90-second rule applies here too — propose the walk without calling it exercise. “Let’s go outside for a few minutes” works better than “it’s time for your walk.” Have a destination, even a minor one — a neighbour’s house, a specific bench, the end of the block.

Morning is usually better than afternoon — energy and mood are typically better before sundowning begins. Ten minutes twice is as effective as twenty minutes once if the full duration is difficult.
